Sporadic Document Failures with RS3 API

Last 2 weeks periodically documents are failing through the RS3 API. Some work, some don't. In our app, the docs work fine and shows they are submitted in the RS Dashboard, but also show "0 of 1 Signature", so no information or signature has been recorded and archived on the document. Tested same docs several times and the tests work fine, but actual employee docs are not going through to RS. Please help!
